Service Description

18 Walker Street, Helensburgh 2508 Provides individual social support services including in home visits, telephone contact, and social activities. Transport for medical appointments, social or general requirements of living. Deliver hot meals, daily if required or frozen on a weekly basis, can also be collected in the office. Have a group social support day program at local clubs on a weekly basis, including cognitive activities, movement for seniors and a lunch. Services are available to people who are 65 years and older, or Aboriginal and Torres Strait Islander people 50 years and over, referred by My Aged Care to ensure independence and would be at risk if placed in premature or inappropriate long term residential care. Community transport is available to individuals (18 years and above) who are transport disadvantage mental, physical and geographical.
