Transitional Aged Care Program (TACP)
Service Description
The Transitional Aged Care Program (TACP) provides short term care that seeks to optimise the function and independence of older people after a hospital stay. It targets older people at the conclusion of a hospital episode who require more time and support to complete their restorative process, optimise their functional capacity, while assisting them or their carer/family to make appropriate long term care arrangements if needed.
